WF just disappeared

I received an Email from a user who says a job is missing and no one can find it (including me). I have checked all the logs I keep as jobs move along and find that it was approved (I log when someone moves a job along in my app). But after the approval - nothing. It just disappeared.

I tried a dummy job and find that if you "remove job" it "just goes away". I can't find any logging that the job was killed or even existed. PLEASE tell me I'm not looking in the right place. It would be rather difficult to explain to my manager that someone can kill a job and there not be a record of that!
