LoadRunner/VuGen Script Playback Problem

Hi. I have successfully recorded a LoadRunner/VuGen script to login to TeamSite, navigate to a folder, import a file, and then submit to a workflow whereby the file is published. No issues recording the script. When I attempt to playback the script, I get a "requested form not found" for the following step of the script:

Why do I get this "form not found error"? It recorded properly. Playback is messed up. The form obviously exists as it was recorded.

Anyone run into this issue using Loadrunner VuGen?

Information for Interwoven Teamsite: Interwoven TeamSite 6.7.1 Service Pack 1 Build: 68040 on Windows Professional XP 2002, SP2.
