blank template page when create or edit DCR

Hi,
We have TS 5.5.2 SP2 running fine. Suddenly we started getting this problem. If we are trying to Edit html (which was generated with DCR) or Create a new DCR....we are getting a blank template page. The blank page has "Teamsite Templating Data Capture Form" as title and its totally blank. I did iwreset -a and even restarted the iwserver. I checked iwui/error_log, servlet_log, etc but nothing significant in there. Please let me know if any one has encountered same issue and resolved. Thanks,
